COVID-19 and INJURY Release

Please note that this release covers all facilities and organization from any risk from injury and or exposure to COVID-19.

You as a parent/guardian voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any illness or injury to our participants, but not limited to, personal injury, disability, and death, damage, loss, claim, liability, or expense, of any kind, that you or our participants may experience or incur in connection with our participants’ attendance while at the Club (“Claims”) on our organization's behalf, and on behalf of our participants, You hereby covenant not to sue, and agree to release, defend, indemnify and hold harmless the Club, its employees, agents, and volunteers, of and from any Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this Liability Release Agreement includes but is not limited to any Claims based on the actions, omissions, or negligence of the Club, its employees, agents, and volunteers. If any provision of this Liability Release Agreement is held to be unenforceable, all remaining provisions shall be given full force and effect.

You, as the parent/guardian, agree to follow the following COVID-19 requirements;

Require all participants submit to Health Screening that includes temperature check and related questions.
Require all participants sign into the building and record required information
All information collected will remain at the Boys & Girls Club
All participants are required to wear a facemask whenever they are not actively competing
Hand Sanitizer should be used frequently
Participants should not share personal equipment

No parents or outside visitors will be allowed in the Boys & Girls Club during the rental program
All participants are only allowed in the approved areas of the Boys & Girls Club
If at anytime these rules change, all parents/guardians within the organization will be notified.
